The former American Idol judge, 69, made a surprise return to the show on May 4, appearing on the Season 24 episode as a guest mentor to the Top 5 finalists. For many viewers, the reunion was a welcome shot of nostalgia. But as footage of Jackson guiding contestants through rehearsal sessions circulated online, a different conversation began to take shape.
Fans watching the episode noticed that Jackson appeared notably diminished — seated throughout much of his mentoring time, and moving and speaking with what many described as visible effort.
“Is Randy Jackson OK? Looks and sounds so weak,” one viewer wrote on X, a sentiment echoed almost word for word across multiple platforms.
On Reddit, the worry was equally palpable: “Randy doesn’t look well. He looks weak and frail. He only sat. I hope he is OK.”
Another added simply:
“Sorry if I missed something about his health — but it’s sad.”
Not everyone shared the alarm. Some pushed back on what they saw as unfair expectations placed on a man approaching 70.

Jackson was diagnosed with Type 2 diabetes in 2002, a turning point that eventually prompted him to take dramatic action. At his heaviest, he weighed more than 350 pounds. He ultimately opted for gastric bypass surgery, shedding over 100 pounds in the process and overhauling his diet and lifestyle in the years that followed.

“For a long time you see me on TV, and people have noticed me in the past, and I was struggling with my weight,” he told People in 2020. “Then I finally got it off by using some pretty drastic measures.”
In 2019, he underwent spinal surgery following a back injury — a procedure that added another layer of complexity to an already demanding physical recovery.
That same year, Jackson channelled his health experiences into entrepreneurship, co-founding Unify Health Labs, a nutrition company aimed at supporting others navigating weight loss and wellness challenges.
“I’m proud to know that we did something that helps me, that helps everyone else, and takes the guesswork out of it for everybody,” he said of the venture in 2022.
He was candid about the stakes involved:
“I think if I had not figured it all out, I would’ve gained at least half, if not more, of that weight back.”
